New Dimensions Caregivers delivers these services through Medicaid-funded programs including PHC, Family Care, Community Attendant Services, STARPLUS, and partnering MCOs.
Eligibility is determined by the Texas Health and Human Services Commission and your managed care organization. Individuals must generally be enrolled in a qualifying Medicaid program and have an assessed need for daily living assistance. Contact us and we can help guide you through the process.
